1.) Never should have climbed above the pivot point (where the ladder meets the roof / gutter line).

2.) Could have had two other fire fighters "foot" the bottom of the ladder

3.) Could have used a ladder stabilizer but with that much weight above the pivot point, I doubt that would have been effective.

This guy also needs a ladder safety course, right Larry  :)

Art is the second guy on the ladder!

Bottom guy foots the ladder,increase pitch of ladder,top guy stays at or below pivot point,tie off center post of garage... use a ladder truck!

They should take a fall arrest safety course!

This was posted last year sometime, I love this video. I laugh so hard everytime I watch it.  To answer the original question.

1. Stabilizer its a $40 no brainer, every ladder should have one. 

2. If your gonna put that much weight up top, you need to have someone down bottom. 

The firefighters could have stayed at a Holiday Inn Express last night.
The QVC guy just needs to learn how to climb down a ladder, one step at a time. Two feet, one hand on the ladder.
