Been on a bit of a hiatus, but I am back in the RCIA saddle again!!! I need to hire an employee for this year and would like your thoughts on it, please.
I would like to hire a guy to drive my truck during the week, and pay him a per job rate (another words, not on salary but he would get paid per job that is completed).
Should he be a 1099 employee? Should I have insurance on him, or because he would be an independent contractor, should I even worry about that?
Anything would be appreciated.

I would be concerned with how he was using the truck, or if he was working on the side. I say that because I don't have enough work to keep a man busy.
Be careful on this one. I'm fairly certain that you are better off paying him as a regular hourly employee and paying into Workers Comp. Even if you employ him as a Independent Contractor if he is hurt on the job site he can file for workers comp. and they will come after you for all medical expenses in this type of situation.

to be an independent contractor he most supply equipment and insurance!! if you supply equipment and in surance... then he is your employee.
be care full with this or you will get in a huge trouble!!! if you are going to train this guy... then make him your employee or you won't be able to make him sign "a non compete agrement"....
Excellent Advice Ivan!!! You are not an independent contractor if you are using the companies equipment. If you can't afford comp (like most exterior cleaners) than use him as a ground man. I would highly recommend you talk to an employment lawyer as well as your local Licensing and regulatory government organization (the guys that sell business license and collect state employment taxes).
